Method of Application
Because there are no restrictions on the strength of marijuana recommended for medical use, a medical marijuana card is required. An AZ medical card can be obtained quickly and completely through teleservice.
- Application - Request a certification from an Arizona-licensed medical professional, who will provide you with a written form to complete. Complete the form and submit it within 90 days of certification to apply for a registry card.
- Validation: After receiving a notification from the registry, you must complete the online patient registration process. You will be required to show proof of residency as well as a photo ID.
